Instructions for Obtaining Your Employee ID Badge

 

New Hires

Once you have completed and turned in all new hire paperwork, email Tara Smith a headshot picture (jpeg format) for the badge and a copy of your driver’s license.  The new badge will be mailed to the employee's work location.

 

Job Location Change or Replacement Badge Due to Loss of Damage

Your principal will need to email Tara Smith the reason for the new badge.  The email must include the following employee information:

Name, Title, School Year IF Part-time or Temporary Placement, Primary Work Location, Reason for the Request

 

The employee will need to email Tara Smith a head shot photo with a plain backdrop (jpeg format) for the badge and a copy of their driver’s license.  Once Tara has received the principal and employee emails, the new badge will be mailed to the employee's work location.  If there is an old badge, it should be returned to Annette Nalbone at Central Office for disposal once the employee has received their new badge. 

 

Name Change Only

For a name change, the employee will need to email Tara Smith a head shot photo with a plain backdrop (jpeg format) for the badge and a copy of their new driver’s license and social security card.  (Teachers will also need to include a screenshot from TN Compass in the email.)  The new badge will be mailed to the employee's work location.  If there is an old badge it should be collected and returned to Annette Nalbone at Central Office for disposal once the employee has their new badge.
